How does payment work?

Payment is due at the time of service via cash, check, or credit card* including flex spending or HSA accounts. We are a cash based clinic and do not have a relationship with any insurance companies.  A "superbill" can be provided upon request that contains all information you will need if you choose to submit for reimbursement to your insurance company. We are considered "out of network" for every insurance company and you can expect to be reimbursed accordingly.

My goal is to get you better in as few treatment sessions as possible. There is no pressure to attend more sessions than necessary.

Do you accept Medicare?

Katie Hoak Physical Therapy, PLLC does not have a relationship with Medicare and cannot accept patients who have Medicare as their insurance because of a law that affects physical and occupational therapy. We can do "wellness visits" for patients who have Medicare which may benefit people who benefit from supervised exercise, maintenance of fitness or strength, or prevention of decline in health.

What is the first physical therapy session like?

1. Schedule appointment during phone consultation

2. Fill out paperwork online via the "New Patient Forms" tab on this website before your first appointment. 

3. Sessions take place in-person at my home in Iowa City. I will meet you at my front door and show you to the private clinic space. We will discuss your medical history and then I will perform a physical therapy examination of the relevant structures. This may include movement analysis, strength and flexibility testing, internal and external examinations. 

 

*All aspects of exam and treatment are optional and will be fully explained to you to obtain ongoing verbal consent. We will only move at a pace that makes you feel comfortable.

What if I'm having my period?

No problem! There is no need to reschedule unless you feel uncomfortable.
